NUCC v26.1 Has 883 Codes and None of Them Is a Med Spa: Auditing a 5,935-Clinic Lead Build Against CMS

A 56-second, 720x1280 vertical clip posted 2026-09-02 at 21:21 UTC by @cfongofficial (channel nickname cfongofficial), scored to "original sound - cfongofficial", carrying 19,600 plays, 949 likes, 37 comments, 269 reposts and 895 saves at capture on 2026-10-05, with hashtags #claude #medclinic #leadgen; I read 24 of the 28 extracted frames plus four upscaled crops, and all 223 transcript words. Frame 1 is a talking-head shot under a white caption box reading "Claude Fable 5.1 + Public Databases = infinite lead glitch 😴💰". Frames 2 through 4 cut to a phone shot of a MacBook screen on hopper-8738f56b.netlify.app, a dark-teal site headed "THE MEDICAL CLINIC LEAD MACHINE" with the subtitle "THE HOPPER" and a nav bar reading "FLOOR LEDGER SLUICE CREW METHOD SOURCES"; a status strip reads "GRADER rubric applied to all 5,935: 1,111 grade A, 2,567 grade B, 2,257 grade C", a pink ticker reads "IF YOU SELL TO MEDICAL CLINICS - BILLING, CREDENTIALING, MALPRACTICE, STAFFING, EQUIPMENT - THIS MACHINE WAS BUILT FOR YOU", and the hero number is "5,935" over "BRAND-NEW CLINICS IN THE TRAY - POURED FROM 105,139 FEDERAL RECORDS" with "WINDOW: Aug 3 - Aug 23, 2026 - the 3 weekly files CMS has published since the last monthly PULL". Frames 6 and 7 scroll to "THE SOURCES", strapline "Free, public, keyless. Every figure on this site traces to one of these in one click", with a panel "NPPES WEEKLY DISSEMINATION FILES - CMS" listing npidata_pfile_20260803-20260809.csv, npidata_pfile_20260810-20260816.csv and npidata_pfile_20260817-20260823.csv above a button "DOWNLOAD.CMS.GOV/NPPES"; a panel "NPI REGISTRY - THE VERIFY BUTTON" reading "CMS's public search. Every dossier links its clinic's full, unmasked record here" above a button "NPIREGISTRY.CMS.HHS.GOV"; a panel "NUCC HEALTHCARE TAXONOMY V26.1" reading "The National Uniform Claim Committee's code set - 883 codes. Every clinic-type label and every 'what this clinic is' definition on this site is quoted from it"; and a panel "THE PULL" listing "PULLED 2026-08-30, morning run", "WINDOW 2026-08-03 to 2026-08-23 (3 weekly files)", "RECORDS READ 105,139", "COST $0 - no API keys, no scraping, public files". Frames 8 and 9 show the ledger, a filterable table with "All states", "All clinic types", "All grades", "All 3 weeks" and "Newest first", rows named "Oneida Speech & Language Services, Pllc", "Pacific Nutrition Collective Llc", "Pangea Hope Professional Clinical Counselor Corporation", "Patient Services Reimagined Corp", "Paz Mental Health Counseling Services Pllc", "Peace Valley Recovery Ii, Llc", "Peter Do Dental Corporation", "Platinum Surgery Center", "Practice Space Therapy Llc", "Prairie Hearts Home Care Llc" and "Proluxe Home Care Llc", each tagged with a city, a "filed 08-2x" date and a letter grade. Frames 10 through 12 open one dossier at /#/lead/1700792405: "PATIENT SERVICES REIMAGINED CORP", "NEW THIS WEEK", "GRADE B - 6 PTS", "Case Management", header line "ON THE FEDERAL RECORD 9 DAYS - filed 2026-08-21, surfaced in the weekly file of 2026-08-17", panel "1 - WHAT THIS CLINIC IS" quoting a taxonomy definition with "Definition: NUCC taxonomy 251B00000X (verify the code)", panel "2 - THE FILING" listing "NPI 1700792405", "ENUMERATED 2026-08-21", "ENTITY Organization (Type 2)", "WEEKLY FILE week of 2026-08-17", "CATEGORY Agencies / Case Management", "Source: NPPES weekly dissemination, download.cms.gov/nppes", panel "3 - WHERE THEY PRACTICE" listing "1225 Franklin Ave Ste 325, Garden City, NY 11530", panel "4 - REACH THEM" listing "PHONE (973) ••••••15 - full number on the public record", "OFFICIAL A. D.", "TITLE Ceo" and the note "Names shown as initials and phones masked by policy - the unmasked record is public at the registry, one click below" above a pink button "VERIFY THE FULL RECORD - NPI REGISTRY", and panel "6 - GRADE B: THE MATH, SHOWN" itemising "phone on the federal record (+2)", "named official on the filing (+2)", "the official IS the decision maker - owner, CEO, president, director (+1)" and "filed in the newest week on file (+1)". Frames 13 and 14 cut to the real CMS site at npiregistry.cms.hhs.gov/provider-view/1700792405, "Provider Information for 1700792405", showing "PATIENT SERVICES REIMAGINED CORP", "Last Updated: 2026-08-21", "Certification Date: 2026-08-21", "NPI Type NPI-2 Organization", "Status Active", "Name: ARINDAM DE / Title: CEO / Phone: 9732161615" and "1225 FRANKLIN AVE STE 325, GARDEN CITY, NY 11530-1693". Frames 15 through 28 are talking head with no overlay.
The dossier checks out: I pulled the same NPI from the live CMS API and every field matched
The strongest thing in the clip is the part it treats as a throwaway. The dossier on screen claims a specific federal record, and that record is queryable by anyone. I called the public NPI Registry API directly with GET https://npiregistry.cms.hhs.gov/api/?version=2.1&number=1700792405 and no credentials of any kind. It returned one result: organization name PATIENT SERVICES REIMAGINED CORP, enumeration_type: NPI-2, status: A, enumeration_date: 2026-08-21, certification_date: 2026-08-21, last_updated: 2026-08-21, authorized official ARINDAM DE with title CEO and telephone 9732161615, mailing and location address 1225 FRANKLIN AVE STE 325, GARDEN CITY, NY 115301693, and a single primary taxonomy 251B00000X described as Case Management.
Every number, name, date and code the dossier displays survives that check. The "ON THE FEDERAL RECORD 9 DAYS" line is also internally consistent: a 2026-08-21 enumeration falls inside the 2026-08-17 weekly window, and 2026-08-21 to the stated 2026-08-30 pull is nine days. The on-screen grade arithmetic holds too, with +2, +2, +1 and +1 summing to the displayed "GRADE B - 6 PTS", and the grader split of 1,111 grade A plus 2,567 grade B plus 2,257 grade C summing to exactly 5,935.
The source panel's claim that this costs nothing and needs no key is accurate as stated. CMS publishes the dissemination files at download.cms.gov/nppes with no registration, login or API key, and the CMS Data Dissemination page explains why: "In September 2007, CMS began disclosing NPPES health care provider data that are disclosable under the Freedom of Information Act (FOIA) to the public."
"Med spas" is the wrong word: the 883-code NUCC set has no such classification
The spoken line is "Found 5,900 brand new med spas I opened up in the last seven days after 105,000 federal records." The screen never says med spa. It says "5,935 BRAND-NEW CLINICS".
I downloaded the code set the site cites, NUCC Health Care Provider Taxonomy version 26.1, effective 7/1/26. It parses to exactly 883 rows, which verifies the on-screen claim "883 codes" to the digit. Searching all 883 rows for "med spa", "medical spa", "medspa", "aesthetic" and "cosmetic" in the Classification, Specialization and Display Name columns returns zero matches. The only row containing the string "spa" is 2083A0100X, "Aerospace Medicine Physician".
The ledger on screen agrees with the code set rather than with the voiceover. The visible rows are speech and language services, nutrition, professional clinical counseling, case management, mental health counseling, recovery, dental, an ambulatory surgical center, clinical social work, registered nursing and home health. The partially legible clinic-type sampler in frame 4 leads with Mental Health Counselor, Clinical Social Worker, In Home Supportive Care and Mental Health Clinic/Center. The sample lead itself is 251B00000X, which the NUCC CSV defines as Grouping "Agencies", Classification "Case Management", Display Name "Case Management Agency". That is the exact text the dossier quotes.
The timeframe in the same sentence is also wrong against the screen. "The last seven days" does not match "WINDOW 2026-08-03 to 2026-08-23 (3 weekly files)", which is a 21-day span. The rounding of 105,139 to "105,000" is fine; the week count is not.
Claude Fable 5.1 shipped on 2026-09-01, two days after the data pull shown on screen
The caption is "Claude Fable 5.1 + Public Databases = infinite lead glitch". Anthropic introduced Claude Fable 5.1 and Claude Mythos 5.1 with the API model ID claude-fable-5-1, and the accompanying system card is dated September 1, 2026. This video was posted 2026-09-02 at 21:21 UTC.
The site's own "THE PULL" panel says "PULLED 2026-08-30, morning run". That data gathering happened two days before the model in the caption was released, so Claude Fable 5.1 cannot have produced the pull the site documents. Whatever part of the build it did touch had a window of roughly 36 hours between the model's launch and this video going live. The clip never claims a build date, so I cannot say which stages used which model; I can only say the pull predates the model.
One capability claim in the caption is worth grounding. Claude Fable 5.1 is positioned by Anthropic as its most capable widely released model for coding and knowledge work, with headline results including 52.6% on Terminal-Bench-Science 0.1. Nothing in the announcement describes a lead-generation or record-scraping capability, and nothing about reading three CSV files requires a frontier model.
An NPI enumeration is not an LLC filing, and CMS says so on the same page the site links
The voiceover's selling logic is "Like they just filed an LLC, so now they're in need of all these different things to make sure that their business actually runs properly." An NPI is a provider identifier issued by the National Plan and Provider Enumeration System. It is not a state business registration, and enumeration carries no implication about company formation date, funding, staffing or operating status. The CMS download page the site links states the limit plainly: "Please Note: Issuance of an NPI does not ensure or validate that the Health Care Provider is Licensed or Credentialed."
The dossier's own footnote is more honest than the voiceover. Frame 10 shows the line "This machine claims the filing and its data - nothing about the business beyond what is on file." That sentence is correct and the spoken pitch contradicts it.
A second caveat the clip skips: CMS keeps only the most recent weekly incremental files on the download page. As of this writing the page lists four weekly files, for 090726-091326 through 092826-100426, alongside the current monthly full replacement at 1,105.79 MB. The three files named on screen are already gone. A build like this has to run on a weekly cadence or re-derive the window from the 1.1 GB monthly file. CMS states the design directly: "the weekly files will only include data for a given week."
The data is free; the pipeline around it is not, and the link on screen is a funnel
The transcript's build recipe is "if you open up an AWS server, which is a cloud VPS server for your cloud code agents, you can deploy the cloud code SDK, plus Google Workspace, SMTP, and now automate email outreach." Whisper misheard "Claude" as "cloud" throughout, and "NPI Registry" as "MPI Registry"; the intended terms are Claude Code agents and the Claude Code SDK. That package name is retired. Anthropic's Agent SDK migration guide states that "The Claude Code SDK has been renamed to the Claude Agent SDK" and that @anthropic-ai/claude-code became @anthropic-ai/claude-agent-sdk and claude-code-sdk became claude-agent-sdk.
Four costs the clip calls free:
Claude Fable 5.1 is Anthropic's top-priced tier at $10 per million input tokens and $50 per million output tokens, with cache reads at $0.25 per million, per the launch announcement.
The AWS Free Tier is no longer an open-ended twelve months for new accounts: "When you create a new AWS Free Tier account, you get $100 in credits immediately. As you explore key services, you can earn up to $100 more. That's up to $200 over 6 months", and "The account closes on its own 6 months after you open it or when your credits run out, whichever comes first."
Google Workspace caps outbound mail well below 5,935 leads. Google's Gmail sending limits are 2,000 messages per day, 10,000 total daily recipients of which only 3,000 may be external, and 100 recipients per message when relayed over SMTP by POP/IMAP clients. Exceeding them blocks sending for up to 24 hours.
The NUCC taxonomy carries "Copyright 2026 American Medical Association" and the download page states that "For commercial use, including sales or licensing, a license must be obtained from this web site." The site republishes all 883 definitions as its clinic-type labels.
The address bar visible in the video is live. hopper-8738f56b.netlify.app now returns a gate page reading "SLUICE GATE CLOSED - THIS PLATE CATCHES THE CURIOUS" and "the hopper moved. The machine you are looking for pours at a new address that only exists off-camera", then auto-redirects after seven seconds to typed-the-link.netlify.app. That page offers three doors: "Have me build it for you", "Learn to build it yourself", and "Join Code Collective", the last linking to a paid Skool community. The build shown in the clip is not reachable at the URL shown in the clip.
Key Takeaways
- Verified: The sample dossier is accurate. The live NPI Registry API returns
PATIENT SERVICES REIMAGINED CORP, NPI1700792405, NPI-2 organization, status active, enumerated 2026-08-21, authorized official ARINDAM DE, title CEO, phone 973-216-1615, taxonomy251B00000X, address 1225 Franklin Ave Ste 325, Garden City NY. No key, no registration. - Verified: "883 codes" is exact. The NUCC Health Care Provider Taxonomy v26.1 CSV, effective 7/1/26, parses to 883 rows, and
251B00000Xis Agencies / Case Management with the definition the dossier quotes. - Verified: The CMS files are genuinely free and keyless. CMS publishes NPPES weekly incremental and monthly full replacement files under FOIA with no login or API key.
- Correction: They are not med spas. The voiceover says "5,900 brand new med spas"; the screen says "5,935 BRAND-NEW CLINICS", and the 883-code NUCC set has no med spa, medical spa, aesthetic or cosmetic classification at all. The visible rows are counseling, social work, nutrition, home health, dental and case management.
- Correction: Not seven days. The voiceover says "the last seven days"; the site says "WINDOW 2026-08-03 to 2026-08-23 (3 weekly files)", a 21-day span.
- Correction: An NPI enumeration is not an LLC filing. CMS states on the download page that "Issuance of an NPI does not ensure or validate that the Health Care Provider is Licensed or Credentialed", and the site's own footnote concedes it claims "nothing about the business beyond what is on file."
- Partial correction: The chronology is inverted. Claude Fable 5.1 was released 2026-09-01; the site's own panel says the data was "PULLED 2026-08-30". The pull predates the headline model by two days.
- Correction: "Claude Code SDK" no longer exists as a package name. Anthropic renamed it the Claude Agent SDK;
@anthropic-ai/claude-codeis now@anthropic-ai/claude-agent-sdkandclaude-code-sdkis nowclaude-agent-sdk. - Unstated cost: Claude Fable 5.1 runs $10 per million input tokens and $50 per million output tokens. The records are free; the agent reading them is Anthropic's most expensive tier.
- Unstated cost: The AWS free plan for new accounts is up to $200 in credits over six months, after which the account closes on its own. It is not a permanently free VPS.
- Unstated cost: Google Workspace allows 2,000 messages and 3,000 external recipients per day, with a 24-hour lockout on breach. Emailing 5,935 leads takes at least two days of sending headroom and risks the cap.
- Unstated cost: The NUCC code set is copyright 2026 American Medical Association and requires a license for commercial use, which a lead product built on its 883 definitions plausibly is.
- Unstated cost: The URL shown on screen now gates to a funnel offering done-for-you builds and a paid Skool community. The "free sauce" named in the voiceover is the CMS and NUCC data, not the tool.
- Caveat: CMS retains only the latest few weekly files. The three files named on screen are no longer on the download page, so this build has to run weekly or rebuild from the 1.1 GB monthly full replacement.
